Jeremy is survived by his wife: Kate Moskodauz, Brice, his children: Leah Moskodauz, Madison Moskodauz & Mia Moskodauz, his father: Terry Moskodauz, his mother: Rita (John) David, his grandparents: Richard & Jackie Moskodauz and Homer & Sally Weaver, he is also a dear brother, uncle, cousin & friend. Memorials may be made to a Trust Fund for his Children.
