Jacob Wade Stevens, infant son of Jason and Rebecca Stevens, was born Tuesday, July 31, 2012 and departed this life, on Tuesday, August 7, 2012 at St. JohnÆs Mercy Medical Center in St. Louis, MO. Jacob Wade entered into The Sacrament of Holy Baptism on Friday, August 3, 2012. Besides his loving parents, he is survived by his sisters: Audra and Avey Cusumano, his grandparents: Virgil & Linda Stevens, Pete & Sandra Woolley and Rick Cusumano, his great grandmother: Virginia Hornbustle, his uncles: Jeff (Sarah) Stevens, Gregory (Robyn) Woolley, his aunt: Jamie (Jason) Payne, his cousins: Alexis, Logan, Colton Stevens, Cameryn, Cassen Woolley, plus many aunts, uncles, and other family members and friends. In lieu of flowers, memorials may be made to Jacob's Heart Project.
